  - questions contained within the report
  - layout or design of the accident /incident investigation reporting forms.
  - Procedures, style, and content
The research will be conducted by ISQEM over the next 3 months around the world and the final report will be freely available for everyone to use. If you would like to contribute to this research programme please send in blank copies of your companies accident report forms and procedures etc., to; mike.wilson@isqem.com

All information sent for the research will be kept and treated as confidential. We will not be quoting any company names in our research unless you specifically state that you wish to be mentioned as a contributor.
